Please arrive about 10 minutes early so that you have time to fill out the New Patient forms. Alternatively, complete the form in advance by clicking the link below to download and print our form.

New Patient & Medical History Form

Initial Examination

Your dentist will start by reviewing your medical and dental history and performing a thorough dental exam.

Your appointment will include a thorough examination of your teeth, gums, bite and jaw, an oral cancer screening and any required X-rays.

Treatment Plan

Based on your history and examination, your dentist will develop a long-term, customized dental treatment plan for you. Your dentist will also dedicate time to answer any questions you have about your oral health and treatment options.

Direct Billing to Your Insurance Company

For your convenience, we accept direct payment from most major insurance providers. This means we can submit your insurance claim electronically, which leads to faster reimbursement, without the need for paper forms and signatures.

Any co-pays or amounts not covered by insurance are your responsibility and must be paid at the time of your appointment. 

Before Your Appointment 

We recommend knowing any essential information about your insurance before your visit. Unfortunately, our office does not have access to any specifics of your plan, including basic coverage, maximums, etc. Be sure to bring any relevant information and your policy number with you to your appointment. 

Preauthorization 

We can send information about your treatment to your benefits provider prior to your appointment to confirm how much will be covered. This pre-authorization information will be sent to you directly, not to our clinic. 

Please bring the pre-authorization information to your visit; we can review it with you to help you understand your coverage.
